Please arrive a few minutes early to complete the forms we will need to allow us to begin your treatment. You may also download the Patient Information form and the Consent form that we will need below. Please complete these and bring them with you.

Your initial examination visit will last approximately one hour and will consist of a review of the findings and discussion of a plan to meet your treatment needs . We will also review home care techniques, and our office manager will discuss your appointment needs and answer questions.
